How to search for a provider
Step 1: Search for a provider using any/or all of the
following combinations: Type of Provider, Specialty,
Provider/Clinic or Last Name, City, State, County,
Zip Code, and Language.
Step 2: Press the search button. The Results
Screen will list all available matches based on the
search criteria selected.
