Search Providers

The Provider Search Screen provides options from which the users can customize their search queries.

Step 1: Search for a provider using any/or all of the following combinations:

· Provider last name

· City

· State

· County

· Zip code              

Step2 (Optional): Refine your search by choosing any/or all of the following options:

· Provider's gender

· Provider's discipline

· Areas of interest

Step 3: Press the search button at the bottom of the page. The Results Screen will list all available matches based on the search criteria selected.

Note: If the initial search results exceed 250, only the first 250 will be displayed. Return to the previous screen to further refine your search, or press the reset button to begin a new provider search.
